Please call the elementary office (937-448-2811) during the week of March 13th during regular school hours to register your child for Kindergarten Screening, which will be held on April 14th.
Basic information will be taken at this time, and you may then fill out your forms on the school website under “Final Forms”
The following information needs to be brought to the school office prior to the April 14th Screening:
- Child’s original Birth Certificate
- Social Security Number
- Proof of residency
- Custody Papers (if applicable)
- Parent’s Driver’s License or State ID
